Opportunities at Silver Fox Pharmacy:

The Clinical Consultant Pharmacist provides comprehensive medication reviews for geriatric residents in LTC and consulting services to health care professionals in Long Term Care Homes and Retirement Communities. The clinical pharmacist also assists in developing and delivering education and policy for home staff and residents, participates in medical advisory meetings, reviews drug-related data in interdisciplinary committees, and ensures that pharmacy and nursing services are compliant with clinical best practice and regulatory compliance standards.

Principal Accountabilities

  • Complete quarterly medication reviews for LTC and RH residents at facilities in Southwestern Ontario
  • Liaise with physicians, nursing staff, and management at the home and pharmacy staff
  • Participate in medical advisory meetings, support accreditation process, and work in an interdisciplinary committee
  • Provide clinical education for nursing staff and residents as requested
  • Assist in developing clinical and educational tools and drug information
  • Review and analyze DUR reporting and medication management trends and present to pharmacy staff and interdisciplinary committees within the homes
  • Maintain a current working knowledge of relevant trends, issues, laws and regulations with regards to geriatric pharmacy
  • Collaborate with pharmacy team on special projects as required
  • Ensures pharmacy services are compliant with Ministry standards and works with the homes to ensure they are meeting Ministry standards from a medication management perspective
Qualifications and Skills
  • Licensure to practice as pharmacist in Ontario in good standing; Registered Pharmacist Part A
  • Preferred: 2 years of experience in long-term care pharmacy, Certified Geriatric Pharmacist (CGP), Certified Diabetes Educator (CDE)
  • Excellent written & verbal communication, interpersonal and presentation skills
  • Proven analytical and problem-solving experience
  • Valid Ontario driver’s license, with good driving record and ability to travel throughout Ontario
  • Proficient with Microsoft Word, Power Point and Excel; Kroll, SQL, Access an asset
  • Ability to work autonomously and prioritize multiple tasks
